Once we have your information you'll need to fax at least 2 of the following items to qualify please allow 3 full business days to process your application.
- A copy of your Federal Tax ID.
- Business card for your gym or business.
- Gym or school flier.
- Yellow page advertisement for your gym or school.
- Black Belt certificate.
- USA Boxing gym registration.
